How to Use the HSE Observation Report Form

1. Record the Observation

Describe the unsafe condition, unsafe act, or positive safety practice clearly and objectively.

Avoid vague descriptions. Include enough information for another person to understand what was observed and where it occurred.

2. Assign the Priority

Select the appropriate observation category level based on the significance of the issue and the organization’s classification criteria.

The priority level should reflect the significance of the observed condition and the risk it presents. High-risk issues may require immediate action, while lower-risk observations can be assigned an appropriate closeout period based on the organization’s procedures.

3. Add Supporting Evidence

Where appropriate, attach a clear photograph showing the observed condition.

The photograph should help demonstrate the location and nature of the observation.

4. Take Corrective Action

The responsible party should implement the required corrective action within the applicable timeframe.

For serious or immediately hazardous conditions, suitable temporary controls should be implemented without waiting for the formal closeout deadline.

5. Document Closeout

Once the action has been completed, record the closeout details and attach a closeout photograph where appropriate.

The closeout evidence should clearly demonstrate that the identified issue has been addressed.
